Office Policies

Our company can provide you with FMLA documentation if you meet the criteria for medical leave under this Act. It is the sole responsibility of you or your employer to provide us with the documents needed to complete the request. Please Note: Most insurances do not cover the fee to complete your paperwork. Please ask our office for the fee schedule to complete your paperwork.

A signed consent is required if you wish to have copies of your records sent to another doctor or organization.

Our practice participates in many insurance plans. An updated list is available on this web site or by contacting our office. Since each plan has different requirements and coverage limitations and exclusions, it is the responsibility of the patient to understand and meet the requirements of their individual plan. Most patients will have a "co-pay" or "coinsurance" (a portion of their charge which is not covered by insurance).
Those covered by Medicare and some other insurance plans may have "deductibles" as well. Many insurance plans and Medicare do not consider "routine" check-ups (those performed without regard to a specific medical problem) or "screening" laboratory studies to be covered. Co-pays, deductibles, co-insurances and non-covered charges are payable at the time services are rendered.
Our billing staff is available to assist you with questions you may have about coverage conditions and payment arrangements. For your convenience, you may make payments by Visa, MasterCard, American Express, Discover, FSA (Flexible Spending Account), HSA (Health Savings Account) and Debit Cards.

What billing or insurance information will I receive?
You will receive a copy of your bill. After your insurance has paid, we will provide you with a monthly statement of your account.

Why am I getting bills from physician?
Each physician who provides professional services to you may send you a separate bill from his or her office. You must contact the physician's office regarding the bill or contact your insurance for a detailed explanation of your bill. Any bills that you receive from MMHS will be explained to you at your request. You may be responsible for paying these bills as well.

I received a notice that my insurance company has paid on my bill, but I can't understand how they calculated their payment amount, do you know?
If we have received any such information from your insurance company, we'll be glad to share it with you. However, for answers to any questions about insurance payments, deductibles, or co-payments, you will need to contact your insurance company.

PRESCRIPTION RENEWALS

Your prescriptions can be sent directly to your local pharmacy for pickup, or through a delivery pharmacy for added convenience. To learn more about having medications shipped straight to your home or office, please contact your insurance provider.

NO SHOW POLICY

We ask that you please arrive 15 minutes prior to your appointment time for in-person appointments or log in at your specified time for telemedicine appointments to ensure adequate and quality time is spent with your provider. This also ensures that each appointment is on time for every patient before and after you.

If, for any reason, you cannot keep your appointment please notify us at least 24 hours before the scheduled appointment time.
